If you have already made a headcrab 2.0 then you should know how to affix the hole/mouth. The trick for making a negative space mouth is attaching it to the roof of the head. So before closing up the last stitches sew a piece in between the mouth and roof of head. You may want to experiment with the length of the connecting piece.
